Dark circles under the eyes are one of the most common cosmetic complaints, yet they are frequently mismanaged because the cause is not properly diagnosed. At Vernon, treatment begins with identifying which type of dark circle you have โ because each requires a completely different approach.
The four types of dark circles:
1. Pigmentary (melanin-based): Excess melanin deposition in the periorbital skin, often genetic and more pronounced in Indian skin types. Appears as brown or dark brown discoloration.
Treatment: Topical depigmenting agents (vitamin C, kojic acid, arbutin), Pico laser toning at very low fluence, gentle chemical peels (mandelic or lactic acid).
2. Vascular (blood vessel-based): The under-eye skin is the thinnest on the body (0.5mm). In patients with prominent superficial blood vessels, the purplish-blue venous network shows through. This type appears worse with fatigue, dehydration, or allergies.
Treatment: Topical vitamin K and retinol to strengthen capillary walls, vascular laser treatment, lifestyle modification (sleep, hydration).
3. Structural (volume loss / hollowing): As we age, the fat pads under the eyes shrink and the orbital bone becomes more prominent, creating a shadow that appears as a dark circle. This is actually a shadow, not pigmentation.
Treatment: Tear trough filler (hyaluronic acid) is the most effective solution. A small amount of HA filler placed along the orbital rim instantly corrects the hollow and eliminates the shadow. Results last 12โ18 months. Our doctors use cannula technique for safer, more precise filler placement in this delicate area.
4. Mixed (most common): The majority of patients have a combination of two or three types. This is why a single treatment approach rarely works.
The Vernon protocol for dark circles:
1. Wood's lamp examination to distinguish pigmentary from vascular component
2. Assessment of tear trough depth for volume loss
3. Customized multi-modal treatment plan addressing all contributing factors
4. Staged treatment โ typically start with filler (if volume loss present), then address pigmentation/vascular components
